PLEASE READ AND AGREE TO THE FOLLOWING INFORMATION AND WAIVER OF LIABILITY:  The above information will be used only for notification of future events at Woodlake Baptist Church.  All participants agree to hold Woodlake Baptist Church, its members, staff and volunteers, as well as sponsors and vendors of today’s show, harmless from any loss, injury, illness or damages incurred during this event.  All show and contest winners must be present to claim awards and prizes.  

Entry Fee and Number Assignment: I understand that this is only the registration form. I agree to pay the Entry Fee of a minimum $20 Donation on the day of the car show. Once the fee has been paid, you will be given a number and a paper to put in your car window to display.
